We will be delighted to welcome you as a patient. Registration at our surgery is a simple process.  

An adult member of the family can register their children 16 years and under by providing the required information and signing the registration application on their behalf.

If you live within the boundary area, please complete our new patient registration

Repeat Prescription - If you have a repeat prescription from your previous surgery, please drop this into the surgery so we can add your medication to our records. Also please provide us with a copy of your medication summary from you previous practice. This can be emailed to prescriptions.caldicotmedicalgroup@wales.nhs.uk 

NHS Number - If you are a UK resident, please ensure that you complete your NHS number on the registration form, as the Practice cannot register you unless we have this.  You can use the NHS website to find your NHS number .

Support - If you require any further information regarding registration, please speak to a member of staff.

Disabled Access & Parking

Both our sites are suitable for disabled users.  There are dedicated disabled car parking spaces within the car park, disabled toilets with disabled assistance alerts.  We would ask that you let us know if you need any help, as your disability may not be a visible one.  If you have a specific need then please feel free to contact us beforehand so that we can double check that adequate facilities have been provided.

Adequate car parking facilities are provided at Gray Hill Surgery, Caldicot.  Public car parks are situated next to both Gray Hill Surgery, Caldicot and Dorset House Surgery, Magor.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
